- Your information will be viewed by our orthodontists, oral health therapists, and nurses of this clinic for the sole purpose of managing your care.
- Your information may need to be shared with third parties for the purpose of liaising various aspects of your care and to comply with legal requirements:
- Other health professionals involved in your care, including your general dentist
- Radiography clinics for X-ray referrals
- Dental laboratories for appliance fabrication
- Ezidebit if you choose to be put on direct debit
- Your health insurance company
- Government bodies if legally required
- Legally required disclosures (e.g. to a court of law, regulatory body, our insurers)
- Where your child is undergoing treatment, it is our policy to share all health-related information with the child’s parents and/or legal guardians, UNLESS you have circumstances where information cannot be shared (e.g. domestic violence, restraining orders, custody agreements).
- You may withdraw consent at any time for information to be shared with third parties (e.g. changing dentists).
- You can access the information held about you and we will provide this access without undue delay. This access might be by inspection of your orthodontic records at the time of appointment or by forwarding of information. We may require you to make this request in writing if forwarding to a third party.
- We will take reasonable steps to ensure that the details we keep about you are accurate, complete, up-to-date, and relevant to the services we provide. Please help us by updating information as changes occur.
- We will take reasonable steps to protect this information from misuse, loss, and from unauthorised access, modification or disclosure.
- Your information will be stored for the required period (usually 7 years) and then will be destroyed. This is recommended practice to minimise risk of privacy breaches from storing information for extended periods that is no longer clinically required.
- Your photographs, teeth models and/or X-rays may be used for teaching purposes to dental and orthodontic students at universities, and to other orthodontists, specialists and general dentists. We will ensure that your anonymity is preserved when using your records for the purposes of the above. If you do not wish your records to be used, you may withdraw consent at any time. No records will be used for any advertising or promotional purposes.
